Allotment of line code : Multimodel Transport Operator (MTO)
| (i) | Multimodal Transport operator is required to submit the following documents alongwith application on letterhead for allotment of Line code. |
| (iii) | List of documents along with formats is detailed below. |
| (iii) | For extension of existing Line Code the documents whose validity is expiring are required to be submitted alongwith application on letterhead |
List of documents :
| (a) | Original Bank Guarantee for Rs. 2 lakhs to be deposited with O.S.C. Branch. Zerox copy of acknowledgement showing agent code alongwith copy of Bank Guarantee to be submitted. |
| (b) | Certificate of registration (MTO License) issued by D.G. Shipping |
| (c) | Certificate for Incorporation with Memorandum and Articles of Association duly notarized. |
| (d) | Transporters Appointment and Acceptance Letter |
| (e) | Acceptance of full liability for the outstanding dues on account of earlier line, if any |
| (f) | Details of executives |
| (g) | Copy of Firm’s PAN issued by Income Tax Authority |
